Benefits are based on a percentage of dollar expenditures up to a maximum dollar limit per enrolled member per year rather than on the dental procedure provided. The first $200 of dental expense is reimbursed to you or paid directly to your Dentist at 100%. (This allows most or all typical expenses for annual preventive dental care to be covered at 100%). After the $200 has been paid, any remaining dental expense is paid at 50% up to an annual maximum of $1,500 per person. No coverage for bleaching or laminates. The lifetime maximum for orthodontic care is $1,000 for adults and children. All limits are applied on the basis of the plan year that begins on January 1 and ends on December 31. Benefits payable to Dentist or Member depending on your arrangement with your dental provider.
